In accordance with the Personal Data Protection Law (“ KVKK ”), please complete and submit the following application form clearly and fully to our Company, Tuba Karacalı (“ the Company ”), so that we can fulfill your request.

We will respond to your application as soon as possible, and no later than thirty days, depending on the nature of your request. If the information and documents you provide are incomplete or unclear, we will contact you to clarify your application.

Our company reserves the right to request additional documents and information for identity and authorization verification in order to eliminate legal risks that may arise from unlawful and unfair data sharing and, in particular, to ensure the security of your personal data. Our company accepts no responsibility for claims arising from inaccurate or outdated information or unauthorized applications submitted through this form. You are solely responsible for all claims arising from applications that are contrary to law or regulations, misleading, or erroneous. In accordance with the Communiqué on the Procedures and Principles for Applications to the Data Controller, if our response to your application exceeds 10 pages, a processing fee of 1 Turkish Lira will be applied for each page after the 10th page.

In this context, applications to our Company in writing should be made by printing out this form;
